HarmonyCares Hospice is pursuing designation as an Age-Friendly Health System through our accrediting organization CHAP (Community Health Accreditation Partner). By working towards this special accreditation, HarmonyCares demonstrates our dedication to providing high quality, age appropriate care. Age-Friendly Care is focused on the 4M’s these include: what Matters most to you, your family, and caregivers, safely managing your Medications: developing strategies to help you increase mobility around your home safely to decrease falls, and injuries: focus on the Mind and monitoring cognitive functioning as well as watching for signs of depression with aging. HarmonyCares is dedicated to providing respectful care, which is based on your individual preferences, and engages our patients as active partners throughout the course of their care.
